Senior Social Worker
Our client is seeking an experienced and compassionate Senior Social Worker to join their dedicated team. Responsibilities: Conduct comprehensive psychosocial assessments to identify client needs and strengths. Develop and implement individualized service plans in collaboration with clients. Provide counseling and support to individuals and families experiencing crises or life transitions. Facilitate referrals to appropriate community resources, including healthcare, housing, employment, and legal services. Advocate for clients' rights and needs within various systems and institutions. Maintain accurate and confidential client records in accordance with ethical and legal standards. Monitor client progress and adjust service plans as needed. Collaborate with interdisciplinary teams, including healthcare providers and educators, to ensure holistic care. Participate in ongoing professional development and supervision. Develop and deliver psychoeducational workshops and support groups. Stay informed about relevant social policies, legislation, and community resources. Respond to emergencies and provide crisis intervention as needed. Qualifications: Master's degree in Social Work (MSW) from an accredited institution. Professional licensure as a Social Worker (e.g., LCSW, LMSW) is required. Minimum of 5 years of experience in clinical social work, case management, or a related field. Proven experience in conducting psychosocial assessments and developing intervention plans. Strong knowledge of community resources and referral pathways. Excellent counseling, communication, and interpersonal skills. Ability to work effectively with diverse populations and build rapport. Proficiency in case management software and electronic health records. Strong ethical grounding and commitment to client confidentiality. Demonstrated ability to manage a caseload and prioritize tasks effectively. Experience working in a hybrid work model.
